Job Description:
A company is looking for an Associate Care & Wellness Program
Enrollment Specialist.
Key Responsibilities
Provide person-centered care management services to long-term care
insurance policyholders
Conduct telephonic assessments to determine risks and educate
policyholders and caregivers about the program
Collaborate with team members to set up program steps based on
policyholder needs
Required Qualifications
2-5 years of professional experience working with older adults and
their families
College degree or certification in a related field such as
gerontology, nursing, or social work
Experience performing assessments for older adults and
caregivers
Ability to meet weekly productivity standards while maintaining
quality
Good working knowledge of systems applications like WORD and
EXCEL
